Submitting Form & Fees *Please submit this completed form & $20 fee by MONDAY, SEPTEMBER 11th with a copy of your child’s baptism certificate with Notations. (Copies only!!)
Meeting Reminder *Mark your calendar for MONDAY, SEPTEMBER 11th from 7-8 p.m. for a parent meeting in Our Lady of Knock Hall (Parents ONLY).

I give permission for my child to take part in the St. Patrick Youth Program activities on and off church grounds. In consideration of the opportunity for my child to participate and fully recognizing that such an undertaking involves an element of risk, we assume all risks and hazards incidental to such participation and do hereby release, absolve, indemnify, and agree to hold harmless the Diocese of Corpus Christi and St. Patrick Church, its agents, employees and officers, and the chaperones, leaders, organizers, sponsors, and persons transporting our child to and from these activities. Neither the Diocese of Corpus Christi nor St. Patrick Church nor any of said persons shall be held financially responsible for any injury, illness, or death incurred as a direct or indirect result of this activity. I, the undersigned, have read this release and understand all its terms and execute it voluntarily and with full knowledge of its significance. In the event of an emergency and I cannot be contacted, I hereby authorize that emergency treatment may be administered.

There is a $20 Registration Fee. Fees can be paid by Check (St. Patrick Church), Cash or Online.

*You can mail Registration Form and Fee to St. Patrick Church, Attn: Confirmation, 3350 S. Alameda St., Corpus Christi, TX 78411.
*You can place the Registration form and Fee in an envelope marked Confirmation in the Mail Slot located at the bottom right hand side of the Rectory door.
*Registration form may be sent electronically here and Fees can be paid online by going to the St. Patrick Website at www.stpatrickchurchcc.org. Click on the ONLINE GIVING icon at the bottom of the page or the Online Giving Tab at the top of the page. If you do not have an Online Giving account, you will need to create one or click on Quick Give.
